Overview

Sage Navigator, Inc. and our Faith Village Connections Initiative exists to help support caregivers and individuals living with dementia around social engagement and re-entry into their faith community in our African American communities. SageNavigator’s goal is to overcome the stigma associated with a diagnosis of dementia for both the caregiver and the care-receiver. This initiative’s intent is to enhance the quality of life for both – the caregiver and the care-receiver by bring the dementia friendly faith village model to individuals and congregations as a bridge to care partnerships, which enhances the awareness and sustainable supports for our communities. This event is made possible by SageNavigator, AARP GA, & Emory ALTER Program
